I certify that I will abide by the laws the code of ethics and professional and trade practices of the Institute, and attest that I shall adhere to the principles outlined therein: that should I deviate from these principles, I shall be subject to disciplinary action as contained in the code.

I undertake that I shall not use the abbreviations PIID, and make use of the PIID MEMBERSHIP CERTIFICATE, if I fail to pay yearly subscription on due date or if my membership is terminated by the Institute. I accept that the Certificate is the property of the Institute and I shall surrender my Certificate promptly to the Institute, if the Institute calls upon me to do so.

I authorize the PIID to make independent investigations of statements made in this application by me.
